Case Report: Full-Arch Rehabilitation with Dental Implants – Upper and Lower Arches

This transformative case highlights the power of modern implant dentistry in restoring function, aesthetics, and confidence for a patient who had suffered from severe dental disease for decades.

Patient Presentation

Our patient presented with a long-standing history of gum disease (periodontitis), which had led to significant bone loss, tooth mobility, and a failing dentition in both upper and lower arches. She expressed deep frustration, having sought care from multiple dental specialists over the past 15 years without achieving lasting improvement.

Treatment Objectives

  • Address the underlying gum disease and eliminate failing teeth.
  • Provide a fixed, stable solution without removable dentures.
  • Restore function and create a natural, harmonious smile.
  • Build the patient’s confidence and ensure long-term success.

Treatment Planning

After thorough discussions with the patient about the prognosis of her remaining teeth—which was extremely limited due to the advanced gum disease—we decided to pursue a full-arch rehabilitation with dental implants. The patient was particularly adamant that she did not want dentures, as she worked in a client-facing role and was very conscious about her appearance and self-confidence.

Clinical Procedure

Comprehensive Assessment and Digital Planning

Using digital smile design and CBCT imaging, we created a precise surgical and prosthetic plan to ensure predictable implant placement and an ideal aesthetic outcome.

Extractions and Immediate Implant Placement (“Teeth-in-a-Day”)

On the day of surgery, we removed all the failing teeth and immediately placed multiple dental implants in both arches. Temporary implant-supported bridges were delivered the same day, giving the patient fixed teeth and immediate improvement in appearance and function.

Healing and Soft Tissue Management

Over a 12-week healing period, the implants were allowed to fully integrate with the bone. During this phase, the patient continued to enjoy fixed provisional teeth—eliminating the need for dentures altogether and preserving her self-confidence.

Definitive Prosthetic Phase

After complete healing, definitive impressions were taken, and final full-arch bridges were crafted using high-strength ceramic materials. These restorations mimic natural tooth anatomy, providing a youthful, vibrant smile that harmonises beautifully with her facial features.

After Treatment and Ongoing Care

The transformation was truly life-changing for the patient. She was overjoyed with the results, sharing that she finally felt at peace and could confidently interact with people without fear of loose teeth or embarrassment.

Now, two years post-treatment, she continues to attend her routine check-ups, and everything remains stable and healthy—just as it was on the day of treatment. The results have stood the test of time, reaffirming the long-term predictability of implant-based full-arch rehabilitation.

Conclusion

This case demonstrates the powerful impact of full-arch rehabilitation for patients with advanced periodontal disease. By combining advanced digital planning, surgical precision, and modern prosthetic materials, we were able to eliminate years of frustration and deliver a healthy, confident smile that truly changes lives.
